Just a reminder that medications and subsequent Health Forms need to be renewed each school year. If you have not seen your child’s provider to have these forms completed, please call them as soon as possible to schedule an appointment or make arrangements to collect the completed form(s).
Medication drop-off will be by appointment only. Appointments are available on 8/24-25 and 8/28. Medication must be dropped off directly to me; I cannot accept medication brought to me by a student or their teacher. Please click here to make an appointment.

Medications can only be dispensed with a signed physician’s order and signed authorization from the student’s parent/legal guardian. This includes ALL medications, including over the counter oral and topical medications. Medications should only be dispensed at school when it is not possible to reach the desired effect with home administration. For example; antibiotics are often ordered for administration three times a day. This can be achieved with home administration of before school, after school and at bedtime. ALL medications must be in their original containers with the prescription label affixed to the either the box or the bottle. Sunscreen is not an approved item to be applied by the teacher or the school nurse without written authorization from a physician.
The following are guidelines set up by the Trumbull School System regarding illness, both at school and at home:
Fever: If a student has a temperature of 100 degrees or higher, with or without symptoms, the student must be excluded from school until they are fever free for 24 hours without the use of fever reducing medication (i.e. Tylenol or Motrin).
Vomiting: Students must be excluded from school and may not return until 24 hours after the last incidence of vomiting.
Diarrhea: Students must be excluded from school if he/she experiences more than one episode of diarrhea. The may return 24 hours after the last incidence of diarrhea.
Please keep your child home if they experience any of the above symptoms during the night or in the morning before school. Your child may appear to be “fine,” but may experience symptoms later in the day and/or continue to be contagious. For the consideration and safety of other children and for staff, please follow these important medical guidelines.
Please contact the nurse if your child has been diagnosed with a communicable disease such as covid-19, influenza, strep throat or conjunctivitis (pink eye).
Please contact the nurse if your child has lice.
